The initial name of a document is now stored in the documents metadata. This
will allow some new features in future.

### Bug Fixes ###
3481326 - Fixed problems with replacing the ParentUUID with a new ParentUUID in
meta.xml.

Made many other changes to import script to make sure that metadata is handled
correctly and no left over junk tags exist.
